I believe the only combined champion title is Dressage/CC/SJ, currently. I propose:

Driving Triple Champion: Driven Dressage + Marathon Driving + Obstacle Driving (this is Combined Driving)
Cow Work Dual/Triple Champion: Cutting + Working Ranch (+ Reining)
Racing Triple/Quad Champion: Sprint Racing + Steeplechase + Racing(+ Endurance)
Gaming Dual Champion: Pole Bending + Barrel Racing
Over-Fences Dual/Triple Champion: Hunter + Jumper (+Steeplechase)
Western Dual Champion: Western Pleasure + Western Trail
Endurance Dual/Triple/Quad Champion: Endurance + Cross Country (+ Marathon Driving + Steeplechase)
Dressage Dual Champion: Dressage + Driven Dressage

There are actually 4 combined titles currently:

Dressage/Cross Country/Show Jumping
Driven Dressage/Marathon Driving/Obstacle Driving
Cutting/Reining/Working Ranch
Steeplechasing/Racing/Sprint Racing

I may consider adding in a few more in the future though :)
